New advance care planning guide for dementia published

NHS England have published a new guide on advanced care planning for people with dementia.

The resource, entitled My future wishes - Advance Care Planning (ACP) for people with dementia in all care settings, is aimed at practitioners, providers and health and social care commissioners, to help them create opportunities for people living with dementia to develop an Advance Care Plan.

The guide identifies actions  from the point of the initial diagnosis of dementia through to the advanced stages, in order to highlight and prompt best practice regardless  of the care setting. It also considers situations where it has not been possible to initiate  conversations about end of life plans, and provides tips on how to manage this.
