Health PEI Engages Their Health Professionals in Advance Care Planning and Goals of Care Discussions

The Provincial Integrated Palliative Care Program team at Health PEI continues to raise awareness of the importance of health professionals engaging in advance care planning and goals of care (ACP/GOC) conversations with patients.

The team recently developed a Palliative Champion group, who meet every six weeks; in Feb there was a presentation on ACP/GOC and on April 11th they will cover the Serious Illness Conversation Guide. 

Sharon Ruckley, a provincial palliative care resource nurse, completed an evaluation and review of a nurse ACP practice project; she invited a sample group of nurses, who had previously attended ACP/GOC education to complete a questionnaire, to determine if improvement in practice had been realized, one year following the delivery of education around ACP/GOC. As part of this project, she also reviewed analytics of both the Health PEI ACP webpage and Speak Up PEI webpage. She found a clear improvement in both awareness and engagement following the ACP/GOC education session. An ACP education booth will be set up at the next Nurses Week breakfast in early May.

The Integrated Palliative Care Program is also running an internal initiative to raise awareness of National Advance Care Planning Day on April 16th. Staff have been invited to take part in a contest encouraging them to compete their own ACP workbook, and win a day off work!! The flyer states:

Complete your own Advance Care Planning Workbook and fill out a ballot
for your chance to win 1 day off with pay! Contest is open until April 13th.

Draw will be noon on April 16th, 2018 National Advance Care Planning Day.

One ballot/entry per staff.

In addition, a Speak Up poster "Why should you encourage advance care planning conversations?", adapted to PEI, will be posted in the washrooms of a local acute facility to coincide with National ACP Day.
